Insurance

OREGON - We are contracted with non-OHP PacificSource, Moda, Regence Blue Cross Blue Shield, and FCH insurance.

CALIFORNIA - We are not accepting new patients in California. Existing California patients may continue to use Lyra, Aetna, Cigna, and United Healthcare / Optum.

For all other PPOs, I am considered an out-of-network provider. I can provide you with a detailed service receipt to obtain reimbursement for my services.

Payment

Most of my clients pay a small co-pay or co-insurance fee per visit. Payments are due at the time of your visit.

Private-Pay or Self-Pay

  • 40-minute Initial Evaluation, Ongoing, or Crisis Session: $500

  • 20-minute Ongoing Session: $250

Telephone & Text Messages

  • No fee for up to five brief calls and text messages per month. Maximum 5 minutes each. 

  • Calls/texts that exceed 5 minutes and/or beyond five brief calls/text messages per month will be prorated at the hourly rate of $500.

Form Fee Guidelines

  • Any letters, forms, or documents that you need me to complete are done outside of scheduled appointment times and may be subject to an administrative fee. 

  • These may include prior authorization appeals for medications not on your insurance formulary. 

  • Payment is due in advance. 

  • Fees start at $15 minimum and can increase depending on the complexity and time involved to complete.

Complete Payment Policy

Please note that the continued availability of my services is contingent upon your being current on your account. Absent extenuating circumstances, I will not see you if you have not paid me in full for any and all services rendered by me.
