You are willingly and voluntarily making this appointment for yourself. 

Please do not make an appointment for someone else.

You have commercial (non-OHP) Moda, PacificSource, Regence, Blue Cross Blue Shield, or First Choice Health Network (FCHN) insurance.

Feel free to schedule while we verify your insurance. Should NP Lee be out-of-network, we can cancel the appointment without penalty.

You understand that we require a valid credit card on file for out-of-pocket expenses, such deductibles, copay, coinsurance, no show, and late cancellation fees.

You understand that NP Lee sees patients for these issues (and may or may not be able to address other issues):

• Alcohol / Substance use / Addictive behavioral issues

• Attention-deficit/hyperactivity disorder (ADD/ADHD)

• Anxiety (panics, social anxiety, generalized anxiety)

• Bipolar disorder or mood instability

• Binge eating

• Depression, sadness, not finding joy

• Insomnia, sleep disturbance

• Obsessive compulsive thoughts and / or behaviors

• Post-traumatic stress

• Side effects of medications, and/or problems with withdrawals

NP Lee mainly offers diagnostic evaluation and/or medication management services. Brief psychotherapy may be offered only in addition to these services.

NP Lee is not accepting new patients solely seeking psychotherapy. 

NP Lee does not support disability or leave applications for new patients. 

You are willing to see NP Lee mainly via telemedicine.

You are ALSO willing to come for rare in-person evaluations at the office, if requested. Note: Such requests would be rare.

Please note that while NP Lee may prescribe benzodiazepines and "z-medications," he ONLY does so for short-term, judicious as-needed use; and NOT for daily use (unless as part of a strict taper protocol.)

Benzodiazepines are anti-anxiety medications that usually end in -azolam or -azepam). (Click here for examples).

"Z-medications" are benzodiazepine receptor agonists (BZRAs) used as hypnotic/sleep medications.
